The requirements to participate in an adult foster care program:
- Have MassHealth of CommonHealth Insurance
- Have a medical diagnosis or need assistance with one or more activities of daily living (ADLs), such as bathing, dressing, eating, toileting, transferring (in/out of bed; up/down from a chair), and ambulation, or management of behaviors such as wandering or inappropriate/disruptive behaviors
- Need ongoing care and supervision
- At least 16 years of age
- Qualified Caregiver-cannot be spouse, parent of minor children, or other legally responsible relative
- Qualified Setting in the state of Massachusetts
- Signed order for AFC services from your primary care physician
- Client must have a recent physical (within 3 months)
